This is a contract between Unleashed® Behavior and Training Services (UBTS) and the above named/below signed person (Owner). In signing this contract the Owner agrees to leave his/her above-mentioned dog (Pet) in the care and tutelage of UBTS for the purpose of training in the following program:
Boarding - $35/night
***OWNER AGREES TO CALL UBTS FOR AN APPOINTMENT IN PICKING UP OR DROPPING OFF HIS/HER DOG(S).
If no appointment is made and Owner attempts to drop off or pick up his/her dog, or if the appointment is missed by more than an hour, a fee of $35.00 will be assessed to Owner.
*Owner agrees to allow UBTS, at its discretion, to seek veterinary care for Pet. UBTS agrees to attempt contact of the Owner before making such a decision. Owner agrees to pay all costs associated with Veterinary care of Pet.
*Owner agrees to hold UBTS harmless in case of illness, injury of loss of pet while under UBTS' care. UBTS agrees to exercise reasonable care for the Pet. It is expressly agreed by Owner and UBTS that UBTS' liability shall in no event exceed the current market value of this breed of dog or the sum of $200.00 per pet boarded.
*Owner understands that training results may vary depending on his/her dog's personality/stress/prior training/experiences etc…. Owner also acknowledges his/her responsibility to follow through the training program as indicated by UBTS. Owner acknowledges that any deviation from said program and/or Owner's lack of involved follow through may diminish expected results.
*Any controversy or claim arising from or related to this contract or the breach thereof, or of the result of any claim or controversy involving the alleged negligence by any party to this contract shall be settled by arbitration in accordance with the rules of the American Arbitration Association and judgement upon the awards rendered by an arbitrator may be entered into any court having jurisdiction thereof. The arbitrator shall as part of his award determine an award to the prevailing party of the costs of such arbitration and reasonable attorney fees of the prevailing party.
